1.1. To demonstrate high-level communication skills.
1.2. To utilise highly developed interpersonal skills including specific
skills to facilitate effective communication when there are barriers
to understanding e.g. sensory or cognitive impairment or in a
hostile, antagonistic or highly emotive situation.
1.3. To establish and maintain robust communication networks with
clients, carers, and other health care workers and numerous
external agencies.
1.4. To establish and maintain good working relationships with GPs, GP
commissioner’s and relevant consortia.
1.5. To promote awareness of the role of CMHT within local service
area/team, GP practices and the wider community.
2.1. Be a full member of the multi-disciplinary team (MDT).
2.2. Effectively manage own caseload accepting referrals from agreed
agencies in accordance with agreed operational procedures.
2.3. Undertake formal mental health and social care assessments of
new clients within own caseload.
2.4. Undertake assessments of clients as part of the CMHT duty system
in accordance with agreed operational procedures.
2.5. Assess, care-plan, provide interventions and evaluate and record
community mental health and social care delivered to clients on
own caseload.
2.6. To provide effective care coordination to an identified caseload of
complex service users, ensuring the delivery of evidence based
interventions to encourage successful outcomes under the Care
Programme Approach (CPA).
