This is an excellent opportunity for our Clinical Transfer Coordinator to be an on-site expert within partner trusts, responsible for the smooth and timely transfer of patients. Maintaining exceptional continuation of clinical care, you will be supporting patients to receive care in their homes and expedite their recovery. Operating with a virtual ward, you will proactively advocate transfer of patients in order to maintain 100% occupancy and maximise bed nights for the saved trust.
